Treatment Program Behavior Contract

This behavior contract between the RDAP Treatment Team and RDAP Resident
Has been established to address the following RDAP program rule violation(s)
and attitude/behavior that can result in this RDAP Resident being an
unsuccessful discharge from the RDAP Program.
The conditions of this contract have been assigned by the RDAP Treatment
Team staff to assist me in focusing on and understanding changing the
behaviors listed below that are impacting/barriers to my recovery process.
