III. PROBLEM PRESENTED

 The Mother of Helen died last 6/5/2017 due to Hepatic Malignancy; Cirrhosis of the
Liver. As the beneficiary of the program, Helen must replace his Mother as grantee
in order for her to receive their grant on succeeding OTC payout.
 IV. BACKGROUND INFORMATION:

Client is living only with his father and since his father died already he is living with
his other relative now. All his siblings are working in Manila that is why they are
planning to transfer Dexter in Manila the next school year.

Dexter is studying in Mambagaton Elemetary School as grade 6 pupil. He is

dedicated to his studies and was never marked as non compliant. His father works
hard for him and attends to his needs before he died.

V. CURRENT SOCIO-ECONOMIC SITUATION

Rolando, his father works as a seasonal farm laborer in sugar-farm lands in Brgy.
Mambagaton City. He never fails to give his son his needs. Their house is surrounded
with sugarcane near Mambagaton National High School.

They have a good health condition and occasionally being sick but manageable by home
remedy. Health center is also near them which made it easy and accessible for them
whenever they got sick. It so happened only that the incident before he died happened
early in the morning around 2am which soon he died because of late application of
hospital care.

VI. Assessment
Change Grantee should be applied to Dexter since his father died already.
