Subject : Request for Untagging Abandonment in the E2M System of shipment covered
by Bill of Lading No. 1430-0645-902.011

Dear Sir/Madam:

Undersigned is respectfully for untagging of abandonment in the e2m system of the shipment
consigned to POLYLITE INDUSTRIAL CORPORATION covered by Bill of Lading No. 1430-0645-902.011
which arrived at the Manila International Container Port on April 07,2019 on board the vessel “YM
WHOLESOME V.0018”, with Reg No. MSK-0037-19 and in support under oath, thereof, hereby depose
and state that:

1. The shipment was tagged as “abandoned” in the e2m system for failure to file the goods
declarartion within the prescribed because the processor of the broker unceremoniusly left
the company without turning over the shipping documents. It was only May 03, 2019 that
the company was able to locate the said person and to retrieved the said import documents;

2. Thus, the reequest for untagging of abandonment falls under Section 3 of CMO No. 24-2015

3. The request for untagging is not intended to violate or circumvent any laws, rules and
regulations;

4. The consignee undertakes to take full responsibility for whatever consequences or liablity
that may resutl from the approval of the request and the release of shipment;

b. Invoice & Packing list
