SUBJECT : IDELINES ON
SURVEILLANCE OFFICERS FOR THE REGION
With reference to the above-mentioned subject, attached herewith are the guidelines for
the hiring of disease surveillance and response officers of the DOH Regional Offices for the
implementation of the Elimination of Diseases as Public Threat programs. Funds to be utilized
for this support shall be in accordance to the issued sub-allotments to regional offices as
indicated in the Department Order 2015-0026.
Dissemination of the information to all concerned is requested.

OFFICERS
Qualification Standards
Graduate of any 4-year academic course or its equivalent
Computer literate and proficient with basic program applications (MS Excel, MS
PowerPoint, and MS Word)
With 2 years relevant experience in Public Health, Surveillance, epidemiology or
biostatistics
With 2 years relevant experiencein drafting Health Policies and Guidelines
Job Description:
‘The staff will be hired and will work directly under the Regional Infectious Cluster in
collaboration with the different Regional Coordinators with disease elimination
initiatives such as Malaria, Filaria, Schistosomiasis, Leprosy and Rabies
Responsible for the verification, confirmation and validation of entries in different
surveillance forms
Coordinates with different surveillance units in local, regional and national level in
relation to the surveillance data
Assists in preparation and dissemination of weekly/monthly/annual disease
surveillance reports
Accomplish and submits Surveillance Reports to DOH stakeholders
Stores documents/forms in appropriate locations
Assists in epidemic/outbreak investigation conducted by PESUs, RESUs or NEC
Assists in conduet of workshops, trainings
Collection of epidemiologic data
‘Data Management (Encoding, cleaning, merging, ete)
Review of guidelines, case investigation forms, etc,
Participates in workshops, seminars, training, scientific meetings and other
surveillance-related activities
Attends advocacy meetings, workshops, trainings within and outside NEC as may be
required
Provides updates to the concerned departments/oflices regarding the status of Disease
Surveillance locally and worldwide
Maintains operations by following policies and procedures
‘Maintains and protects operations by keeping information confidential.
Reporting of project expenses on a monthly basis
Does other related work as assigned by the unit head.
Recommended Rate:
Php 18,000 — Php 25,000.00/month x 13 months
