and further submit to AMO Secretariat office Bureau of Customs our company
additional list of importables.

2. THAT, in compliance per customs regulations list of importables with general terms,
Tariff heading, technical description, volume is hereby submitted

3. THAT, I am executing this affidavit in order to attest to the truth of all the foregoing
Facts and to enable us to facilitate the immediate release of our shipment from the
Bureau of Customs
