ABSTARCT

PT. PLN (Persero) as a State-Owned Enterprise (BUMN) engaged in electricity,

namely providing and distributing electricity to consumers. In distributing electricity PT. PLN
(Persero) uses a transformer. There are 2 types of transformers used by PT. PLN (Persero) in
distributing electricity, namely 3 phase transformer and 1 phase transformer. The type of 3
phase transformer is mostly used for industrial and office needs that require large amounts of
power, while 1 phase transformers are used for household, shopping, and small industries that
require little power and other reasons because the costs required are relatively cheaper.

PT. PLN (Persero) ULP Kartasura has consumers who are mostly household
consumers (housing, rural), shops, and small industries, so ULP Kartasura uses a lot of 1
phase transformers in distributing electricity. Because of several factors causing the 1 phase
transformer to experience interference and even damage, so the transformer cannot be reused
and replaced. The transformer replacement are carried out in accordance with the provisions
contained in the applicable Operational Standard (SOP) to minimize work accidents. And
