COMMISSION ON HIGHER EDUCATION ee — REGION X a Cagayan de Oro City bon “otetax: 856-4380; Emal:chodio0@ched cov oh REGIONAL MEMORANDUM Now, Series of 2915 To 2: Presidents/Heads of Public and Private Higher Education Institutions in Region 10 SUBJECT : REITERATION OF GUIDELINES ON THE GIVING OF STUDENT GRADES AND SUBMISSION OF PROMOTIONAL REPORTS DATE 2 February 6, 2019 In accordance with the pertinent provisions of Republic Act (RA) no. 7722, otherwise known as the "Higher Education Act of 1994", the Commission on Higher Education (CHED) Regional Office X hereby reminded the Higher Education Institutions (HEIs) in the Region through the Office of the Registrar to strictly implement the herein provisions of CMO 40, s. 2008, entitled "Manual of Regulation for Private Higher Education of 2008", to + Section 109. Requirement for Grading. The grading system for in curricular or component subjects/courses of any degree program shall strictly comply with conditions or requirement as follows: * No provisional, conditional, or temporary final grade for any curricular or component subjeci/course shall be given to the student. In case a student fails to take a final examination or submit an academic requirement for completion of a subject/course and that his/her scholastic performance is not sufficient to merit a final passing grade, an institution may, consistent with its academic policies, give a student — = A final grade which does not earn any academic credit nor indicate failure such as "NC" for "No Credit" or "NG" for *No Grade". Such a grade is permanent and cannot be subsequently changed. * Provided however, that where the failure to take the final examination, or to submit the academic requirements, is due to excusable grounds, such as sickness, emergency, or accident, the student maybe given an incomplete mark or "INC". Provided further, that the institution allows special or completion examinations, or additional time for compliance of the requirements. In no case shall an incomplete or "INC" mark remain for more than one (1) academic year. Pursuant to the above provisions, all student academic grades upon submission of the schoo!'s promotional report to CHED Regional Office shall be considered final, thus oa Submission of Schoo! Documents roe | AJA 1. Only PROMOTIONAL RECORDS submitted without BLANKS or NO ENTRY in the FINAL GRADE will be accepted 2. HEIs are given until March 1, 2019 to rectify all its past submissions with blanks or no entry as final grade. Thereafier all records already submitied shall be deemed FINAL 3. HEIs are to re-orient its faculty on this matter to correct the misconception and discourage the continued GIVING OF “INC” GRADES without excusable grounds Moreover, any request for change of grades with mark "INC" to passing grade after the one (1) year validity period will no longer be entertained by the CHED Regional Office. Request for change from "INC" to passing grades within the prescribed period will be considered provided there is valid reason/s and the following documents are submitted: 1. CERTIFICATION UNDER OATH from the Teacher In-Charge that the student (who was under excusable circumstance) has already passed / completed the subject, 2. LETTER FROM THE TEACHER IN-CHARGE ADDRESSED TO THE OFFICE OF THE REGISTRAR REQUESTING FOR THE CHANGE OF GRADE WITH MARK "INC" TO PASSING GRADE duly recommended by the Dean and/or Department Head and approved by the VP for Academic and/or College Dean, and 3. REQUEST LETTER FROM THE REGISTRAR'S OFFICE to CHED Regional Office for the change of grade/s together with the above-mentioned documents.
