AFFIDAVIT OF NON-COLLECTION OF FEES

REPUBLIC OF THE PHILIPPINES)

MUNICIPALITY OF TANAY )S.S.

I, __________________, of legal age, Filipino, single, a Faculty member and

NSTP Coordinator of ___________________________________________________,
a higher educational institution at ____________________________________, after
having been duly sworn in accordance with law, hereby state:

1. That said higher educational institution offers the course subject, National
Service Training Program (NSTP) to all its students who don’t have yet the
credit of taking the subject. That two (2) of its components, namely, Civic
Welfare and Training Service (CWTS) and Reserve Officers Training Corps
are being offered.

2. That said aforementioned institution does not collect any NSTP fees including
the basic tuition fee, which is not more than fifty percent (50%) of what is
currently charged by schools per unit, that being stipulated in Sec. 8 of RA
9163.

3. That since there was no collection of fees, there will be no expenses report
from the NSTP fund which should come from the collected tuition fee.

4. That I execute this Affidavit to attest and declare the truth of the foregoing
facts and for whatever legal purposes that this affidavit may serve.
