Description
On 24/05/2019 0745 hr, at PVC III Expansion Project MCC building, Shakeel Ahmed Scaffolder was
observed working at height, while erecting Scaffolding, no hook with safe anchorage point, falling
hazard may occur, and lead to high risk of fatality. Counseling about scaffolding and safety
harness was done at site.

Immediate action Taken

Stop the work.
Counseling about Personal fall arrest System as Safety Harness and hook to proper anchorage point.
Terminated from site permanently.
