MEMORANDUM OF UNDERSTANDING
KNOW ALL MEN BY THESE PRESENTS:
This Memorandum of Agreement entered into and executed this ____________
in Cagayan de Oro City, by and between.
the COLLEGE OF COMPUTER STUDIES, a college of XAVIER
UNIVERSITY, a non-stock, non-profit corporation duly organized and
existing by virtue and under the laws of the Philippines, with its principal
office located at Corrales Avenue, Cagayan de Oro City, herein represented
by its dean, GERARDO S. DOROJA and herein after referred to as “XU CCS”.
and
_____________________ a corporation organized and existing under
the laws of the Republic of the Philippines, with principal place of business at
_____________________ herein represented by its
_____________________, _____________________ and herein
referred to as "__________________".
WITNESSETH: That -
WHEREAS, XU CCS and __________________ recognize the importance of
developing students’ proficiency in the basic information technology skills;
WHEREAS, for a more effective implementation of the supervised field experience
program, it is important that all parties concerned understand their duties and
responsibilities; and perform them well.
NOW THEREFORE, for and in consideration of the foregoing premises, the parties
hereby agree, as follows:
A. Responsibilities of XU CCS:
1. To coordinate with __________________ regarding placement of students
enrolled in the program;
2. To coordinate with __________________ personnel-in-charge regarding
program activities of students;
3. To conduct regular visits of students in __________________ to see that
objectives of the program are being achieved;
4. To provide __________________ where the students had their OJT a copy of
the students’ program report; and
5. To secure the necessary permits or licenses from government agencies, if
appropriate and the consent of parents of the students involved in the Program.
B. Responsibilities of __________________:
1. To expose students to its day-to-day business operations that will make possible the
fruitful accomplishment of the objectives of the program;
2. To the extent practicable, involve students in hands-on experience so that they
develop the basic Information Systems skills necessary for self-productive
employment;
3. To provide supervision to students undertaking the program. XU CCS and the
students hereby free and release __________________, its officers, personnel
or employees in case of any claim, damages or interest of whatever nature arising
from the implementation of this Agreement;
4. To discuss with students and instructor-in-charge problems that may arise from the
student’s OJT activities and work out with them solutions to the problems.
5. To evaluate the accomplishments of students who have undertaken OJT using a
preformatted form and discuss this evaluation with the student and the instructor-
in- charge.
The students or XU CCS shall not disclose any confidential information they may
have acquired from __________________ during the students’ tenure in the
practicum. “Confidential Information” means information not generally known to third
parties and which is proprietary to __________________.
This Agreement takes effect immediately upon approval by both parties and shall be
accordingly enforced. It shall expire upon completion of the OJT of the students. This
Agreement may be pre-terminated by either party by providing written notice thirty (30)
days prior to the effectivity of the termination.
IN WITNESS WHEREOF, the parties hereunto affix their respective signatures
