Good day!

I am Josette R. Barnedo, incoming 2nd year college student taking up BS Food Technology at
Polangui Campus. I am residing at Sto. Domingo, Albay. The distance from home to school is giving me a
hard time coping up with punctuality and is affecting my studies also, my family is experiencing hardship
in paying for my board and lounging. Because of this, I desire to shift to another course in the main
campus which is Bachelor of Science in Exercise and Sports Sciences.

On this note, may I request that I may be granted to shift in the course that I desire to take. With
this, I am sure that I will be able to focus on my studies since tha main campus is much closer to my
residence area.

I do hope that you will grant my request.
