A PUZZLING FINDING IN ECHOCARDIAGRAPHY: RUPTURED SINUS

OF VALSALVA ANEURYSM

ABSTRACT

A 67-year-old male presented with drowsiness, altered sensorium, and


progressive breathlessness from few days. There was past history of DCMP,
severe LV systolic dysfunction, atrial fibrillation, TIA and hypertension. On
examination, he had a systolic murmur, elevated jugular venous pressure, and
pedal oedema. The history, clinical examination, investigations initially
suggested as a case of DCMP,AF,CHF,CVA but the strange finding in
echocardiography led to suspicion of alternate diagnosis required further
investigations like cardiac MRI , aortography. This came out to be case of
ruptured sinus of Valsalva aneurysm is discussed along with review of relevant
literature.
