residents of Brgy Ipag, Mariveles, Bataan, after having been duly sworn to in accordance with laws do
hereby depose and say;

That we personally know ARISTON P. SEGUIRAN who was born on February 22,1960 at Brgy Abacan,
Tobias Fornier (formerly Dao) Antique.

That ARISTON P. SEGUIRAN is presently residing and our neighbor at Brgy Ipag, Mariveles , Bataan.

That ARISTON BALLESCAS SEGUIRAN born to mother SALVACION BALLESCAS as per PSA Birth
Certificate.

That ARISTON P. SEGUIRAN born to mother SALVACION P. SEGUIRAN as per SS record.

That ARISTON BALLESCAS SEGUIRAN and ARISTON PACHECO SEGUIRAN refer to one and the same
person born to one and the same mother.

That ARISTON P. SEGUIRAN is using middle name PACHECO since he enrolled in grade one and in all
school records, and private transaction, up to the present.

That we execute this affidavit to justify the facts , That, ARISTON P. SEGUIRAN is using middle name
PACHECO because it is also the same middle name he used when he applied for SS number and
COMELEC Voters ID.
