 Detailed room orientation to patients is one of the important factors which can reduce the
incidence of patient fall but through interviewing patients it has been found that the room
orientation was not up to the mark mainly for Wing A as 18% of the people were not
satisfied and same for Wing B 19% of the people were not satisfied.
 Safety aspects regarding fall prevention were not explained properly in Wing C and
Wing D as 44% and 43% respectively were not aware about the facts of fall prevention.
 39% of wing A and 37% of wing B patients were not aware of emergency call bell in the
washroom which is a major error from the nursing point of view , as from the history of
patient fall it has been recorded that most of the patients fall has occurred in the
washroom, if the patient remain uneducated about this safety aspect the chances of
incidence of fall would increase.
 In all the wings, below 20% patients use railing in the washroom, this points out attitude
and behavioural problems in the patients.
 According to the data patients are more dependent on their family members for
assistance on using washroom and do not prefer nurses aid.
 In all the wings above 60% patient along with their attender have reported that the
patient safety pamphlet has not been provided to them.
 Even if the patient safety pamphlet is provided to the patients and attender, it will less
likely impact the patient fall as majority of the patients and attenders has confessed that
they are not interested to read and follow the tips provided in the patient safety pamphlet.

Recommendations
 An environment safety checklist should be prepared, and rounds should be conducted at
least once in a month so that problems are identified, corrective actions are taken, and it
must be monitored periodically.
 Proper and detailed training should be given to the nursing staff on how to provide room
orientation to the patients.
 To improve communication among staff, patient care signage should be introduced in the
hospital in order to attract the attention of nursing staff so that they can identify high fall
risk patient and provide better care.
 Hide checks must be conducted time-to-time to ensure proper care is given to the patient
by the nursing staff.
 As patients from various region of the country and international patients from middle east
who do not follow English language come to Columbia Asia Hospital for treatment,
communicating with them on patient safety is a barrier, so multilinguistic and easy to
understand pictures and graphic tools should be introduced by the hospital in order to
resolve this issue.
 A modified version of patient safety pamphlet specific to fall should be prepared in
different languages and should be explained properly to the patients.
 Educational video on patient fall prevention can be aired on the TV’s available in
common areas like wards, near nursing station, in OPD and in café, which are only used
for entertainment purpose at present.
 From the survey it is seen that nurses don’t have proper knowledge about fall, so in-detail
continuous medical education (CME) should be conducted.
 After recruitment of new staff the orientation given is incomplete or is delayed for the
new joinee, which is a major issue and this need to be corrected by giving proper and
timely orientation.
 The incidence of patient fall is more during night, but it has been observed that nurses
sleep and if the patient calls for assistance through emergency bell the time taken to visit
the patient room was up to 5 mins, this minimalistic thing need improvement.
 2 times rounding should be made mandatory to patient room at night in order to see all
the things are in place.
 Focus should be given to those patients whose attender are not staying with them due to
any personal reasons.
