Carpenter St. Pursuant to Rule 6 ofthe Rules Regarding the Suspension of Privileges to File fora Building Permit or Other Permit issued by the Department you may request a preliminary hearing to contest the basis for the suspension as follows: within ten (10) calendar days ofthe above Date of this Notice or the date on the proof of mailing, whichever is ater, you may file a written request with the ‘Commissioner for a preliminary hearing to contest the suspension of privileges, \written requests may be fled in-person Monday through Friday, excluding City Holidays, from 8:30 am to 4:30 pm at 121 N, LaSalle St, Room 906 or via email at OBCommissioner®cityofchicago.orx: 2 ‘upon receipt ofa timely written request for preliminary hearing, the Commissioner shall hold a preliminary hearing within two (2) business days, ‘excluding weekends and City Holidays; ‘the person or entity is entitled to appear atthe preliminary hearing to testify, present witnesses, and present any other evidence regarding the petition, and may be represented by counsel atthe their sole expense, and in lieu ofan in- person appearance may contest the suspension of privileges through the submission of written documentation; neither the request for nor the outcome ofa preliminary hearing shall impact or diminish the person or entity's right to seek a fll hearing to contest the suspension or of privileges pursuant to Section 19-6-130(b) ofthe Code. In the event that you file a timely written request fora preliminary hearing, the suspension will be stayed until the Commissioner or the Commissioner's designee Issues a determination at the conclusion ofthe preliminary hearing. You may also request a full hearing to contest the basis forthe suspension and written requests ‘may be filed in-person Monday through Friday, excluding City Holidays, from 8:30 am to 4:30 pm at 121 N, LaSalle St, Room 906 or via email at DOBCommissioner@cityofchicago.org, A Fequest for a full hearing shall not stay the suspension. No new permit applications or pending permit applications for any work at any location other than to correct the bass for the suspension willbe accepted or processed until the basis for the suspension is cortected and verified by the Department. In addition to any other fees and fines, a fee of $150.00 shal be paid to the City for each inspection or review conducted by the City to verify compliance. I you contests the basis for the suspension and do not agree with the final decision of the Commissioner, you may appeal the final decision to a court of competent jurisdiction. If the Commissioner suspends the permit privileges, you may file a written petition with the Commissioner for re-instatement of the permit privileges. The petition shall include proof that the noncompliance has been abated or corrected and that all related fines and fees have been paid, The written petition may be fled in-person Monday through Friday, excluding City Holidays, from 8:30 am to 4:30 pm at 121 N.
