Dear Ma’am/Sir,

I am very sorry to be writing this letter of resignation from my position as

Captain II effective immediately due to my health condition. As you are aware I
have been having some medical problems since last month and it has left me now
unable to continue working.

I have recently undergone surgery for excision of fibro adenoma which

caused me not to be able to lift heavy. I feel that it is best for me and the company
if I take some time to focus on recovering from my surgery and I am unable to do
this whilst I am still working. I do hope that you understand the predicament that
I’m in.

Having been given the chance to be part of the Team is a great honour for me.
Thus, I would like to express my gratitude to the management who has given me
all the support I needed in order to perform my job properly and helped me to
develop my hospitality and customer service skills. I really hope to work here with
you again in the near future with the healthy version of me.

In turn, I would like to wish you all the best for the coming years.
