AFFIDAVIT OF ONE AND THE SAME PERSON

I, MARITESS C. ANG of legal age and a resident of 10 Riverdale St.,

Forest Hills Subd., Novaliches, Quezon City respectively after having
sworn under oath do hereby depose and state the following:

1. That I am the same person of MARITESS C. ANG as

indicated in the Driver’s License No. NO4 -92-215261 as well as in the
National Bureau of Investigation Clearance issued and valid until January
15, 2020 and the the MARITES C. ANG indicated in the DTI Certification
and the Business Permit issued on January 12, 2019 are one and the
same person. See attached photocopy of Driver’s License & National
Bureau of Investigation as ANNEX “A” and as Annex “B” , the Business
Permit and DTI Certification.

2. That the MARITES with single “S” was erroneously written in

the DTI certification as well as the Business Permit issued by Quezon City
when in truth and in fact, my name Maritess is with double S.

3. That I am executing this affidavit to attest to the veracity of

foregoing and for other purpose that it may serve.

Affiants sayeth naught.
