123D CIRCUITS “PAPER TRAIL”

PARENTAL CONSENT FORM

Fill out this form, scan or photograph it, and email it to us at:
123dcircuits.parents@autodesk.com
Your child’s provisional account will expire within 10 business days
from initial creation unless you complete the approval process.

Your child just created a free account through 123D Circuits, and needs your permission to become a full
member of our Project Ignite Community. The Autodesk Project Ignite Community is a collection of applications
that help your child learn about design, 3D printing, electronics, programming, and more through hands-on
projects.

Once approved, your child can create and share projects, post comments, upload a photo, collaborate with
others, contact customer service, and save wish lists for projects and kits. They can also complete projects
through the Project Ignite learning platform, where they can join private classrooms and post reviews. Children
may be able to share personal information while participating in some of these activities but you can log into
your account to see what they’ve been up to, moderate their activity, and change their username as necessary.

We may also collect persistent identifiers to customize your child’s visit or display ads for other age-appropriate
Autodesk applications based on their activity while using our applications. For more information on persistent
identifiers and advertising, click here: http://www.autodesk.com/company/legal-notices-trademarks/privacy-
statement/autodesk-privacy-statement-2014-english#_Information_we_collect_1

We take your child’s privacy very seriously, and work hard to keep our educational community safe and friendly. To
comply with the U.S. Children’s Online Privacy Protection Act (COPPA), we must get parental permission before allowing
kids under the age of 13 to join the Autodesk Project Ignite Community. To see a description of all our privacy practices,

You must complete the approval process below within ten business days of the date on the email you received, or we will
need to close and delete your child’s account. Pending your approval, your child will be limited to taking our tutorials and
creating designs that are kept in a private gallery.

What’s happened so far?

Since your child is under 13, a parent or guardian must approve their account.
• Your child has created a username and provided your email address as their parent or guardian.
• Your child can view projects and explore tutorials while waiting for your approval, but their temporary
account will expire and be deleted along with your email address if not approved within 10 business
days.

How do I approve my child’s account via Paper Trail?

• Create or sign in to your own Autodesk account at http://123d.circuits.io.

• Print out, complete, and sign this form – then clearly scan or photograph the form and email to
123dcircuits.parents@autodesk.com.
• We will hand-process the form within three business days and manually approve your child.

After approval, your child’s account is all ready to go! Give your kid a high-five for being awesome!

If you have any questions or run into bumps, please contact us at 123dcircuits.parents@autodesk.com.
