6:240-E7 – Overnight Accommodations Student/Parental Consent and Field Trip Release Form

Glenbard Township High School District No. 87, DuPage County (“School District”), has organized an
overnight field trip in which students will share accommodations (the “Overnight Field Trip”) the
undersigned student and parent(s)/guardian(s) require that teacher (staff organizers) shall place
students with roommates described below:
