We at LILLY MOBILE CARE are pleased to inform you that we like to offer you a position as MEDICAL
ASSISTANT at our company. We would like you to start on 2 MAY 2019.

Upon acceptance of this letter, the following sets out the terms and conditions of employment that
shall to your employment.

LILLY MOBILE CARE will pay you a consolidated salary of RM 1300 per month and no other allowance
will be paid. This compensation will be adjusted depending and in pursuant to the company’s
employee compensation policies in effect from time to time.

Your working hours will be based on rosters provided by the management and according to the
discretion of the management as long as notice is being imposed and hereby given.

You will be held responsible for carrying out work as per designation of the management with entire
satisfaction and responsibility.
