AFFIDAVIT OF DISCREPANCY DATE OF BIRTH

I, MA. ELENA BERNARDO AROMIN, of legal age, widow, Filipino and a resident of No. 3,
Aries Street, Veraville Homes, Almanza Uno Las Pinas City, Philippines, having been duly
sworn in accordance with law, hereby depose and state:

1. THAT my husband JOEL TRIAS AROMIN and I, Ma. Elena Bernardo Aromin got married
on Aug. 4, 1993 at Municipal of Hagonoy Bulacan. When we received our copy of our
Marriage Contract Certificate we found out that his Birth date appeared was February 25,
1968 and his Birth Certificate appears February 5, 1968 as stated and registered in his
birth certificate issued by office of the Civil Registry of Manila. A copy of his birth
certificate is hereto attached as ANNEX “A”;
2. THAT in his Certificate of Live Birth as recorded in the Office of the Civil
Registrar of Manila ,Philippines and Secondary Student Permanent Record at
University of Perpetual Help High School and Collage , and his documents of
employments it is stated that he was born on 05 February 1968 at Manila,
Philippines;
3. THAT I, his wife acting on the honest belief that he was born on 05 February 1968.
4. THAT I am executing this Affidavit of Discrepancy to attest
to the truth of the foregoing facts and to explain the discrepancy in my husband’s
Certificates of Live Birth, as regards to his date of birth appeared in our Marriage
Contract Certificate and to use the same for whatever legal purpose it may serve.
