SUBJECT : One (1) Desktop Computer set with Printer and Scanner

DATE : 21 November 2018

1. This is to inform you that the computer set that was donated by the Local
Government Unit of Camalaniugan is already unserviceable and is beyond economic repair.

2. We are preparing our monthly reports from available computer shops at the
public market. Hence, it is a burdensome for us to beat the deadline for submission of reports
and preparation of other communications.

3. In this regard, the undersigned humbly request to your good office for a new set
of One (1) desktop computer set with printer and scanner so we could have a prompt and
accurate communication and reports to be submitted.

4. Looking forward for your approval regarding this matter.
