internship/related learning experience/field study, g) academic and
non-academic competitions h) seminars, trainings and
conventions

3) I hereby declare that I am allowing him/her to participate in such

activities provided that the said activities are authorized by San Pedro
College; provided further that I am subsequently provided a Parent’s
Consent Form per activity, which need not be notarized;

4) I am executing this affidavit to attest to the truthfulness of the foregoing

facts, in compliance with the requirements of the Commission on Higher
Education and for whatever legal purpose.
