Good day ma'am!

I would like to ask for a favorable consideration and utmost understanding of your good office
ma'am, given that, we do desire to push through our nutrition month's school level culmination.
The attached program of the month of July was prepared ahead by our school clinic teacher ,
with every teacher's committee help to realize each activities.

With the advance planning, the different contests by grade level and the weekly
implementation of the achieved activities, our pupils have shown excitement and full
cooperation. For the coming culmination program this 26th of the month, we humbly ask
permission from your good office to regard our cause. The culmination contests are the jingle
and wellness dance to be performed by our grades four to six (4-6) pupils. Such young ones
spent their extra time after class practicing their made piece and choreographed it creatively
with teacher-coach's facilitation.

I appreciate much your positive consideration, and look forward to your kind approval in this
matter. God bless always!
