Go to File

last or second last will be options (in other versions it will excel options), click Options
Go to Add-Ins (again from bottom)
You can see Manage.. Excel Add-ins .. Go…

Click on Go…
 You have to all three as shown in figure (if any of these checkboxes
were empty). Press OK. Some configuration process will start. Let it complete.

Then again do the same process and come to the same Add-Ins, and now click Browse…

It will search files, go to the PHStat4 folder.

Select PHStat, and then click OK.

Now you can see Phstat selected, and then press OK.
You can see PHStat under Add-Ins ribbon and/or a new ribbon PHStat.

Both are the same thing with different looks.
