accordance with law do hereby depose and say:

1) That MARIA SUZARAH STO. DOMINGO, born on 22 February 1967, a Filipino

citizen, and a resident of Unit 1827 Tower 2 Grass Residence, Nueva Vizcaya,
corner Misamis St., Brgy. Sto. Cristo, Quezon City, is a holder of Philippine
Passport withP3678148 A;

2) That MARIA SUZARAH STO. DOMINGO will travel with us ro Japan for tourism
purposes from 26 August 2019 to 03 September 2019, or for period of nine (9)
days;;

3) That we are sponsoring her trip;

4) That we are financially capable to support her stay abroad;

5) That we guarantee that she will not violate any existing laws of the country of
destination (Japan); and

6) That we are executing this affidavit to attest to the truth of the foregoing
instrument and for any legal purposes and intent this may serve..
