Parent's Consent

This is to certify that I am allowing ___________________, to attend volleyball trainings starting

July 1-18, 2019 for the preparation for the upcoming JPIA WEEK CELEBRATION to be held on July
19-20, 2019.

It is understood that he/she abides by the rules and regulations that may be imposed by the
school.

I fully agree to waive any responsibility on the part of Negros Oriental State University in case of
any unforward incident that may happen to him/her during the duration of the training.
