PETITION
Undersigned petitioner, to the Honorable Executive Judge of
the Regional Trial Court, Sixth Judicial Region, Iloilo City, most
respectfully state:
1. That he is a citizen of the Philippines, twenty nine (29)
years old, having been born on May 22, 1978 in the City of Iloilo and
a resident of 8th St., Lawaan Lopez Village, Balantang, Jaro, Iloilo City
since 1978 up to the present;
2. That he has satisfactorily completed the required course in
law at the College of Law, Central Philippine University in April 2006;
3. That he has passed the Bar Examinations given in the year
2006, and has been admitted to the practice of law in the Philippines
on April 26, 2007;
4. That he is a member of the Philippine Bar in good standing
with clearances from the Office of the Bar Confidant of the Supreme
Court and Integrated Bar of the Philippines. Marked and attached as
Annexes “A” and “B” respectively, are said clearances to be taken
as integral part hereof;
5. That he has not been charged in the first instance of any
crime involving moral turpitude;
6. That he has paid his 2007 Professional Tax as shown by
Official Receipt Number 2543572 issued on May 18, 2007 in Iloilo
City, a copy of which is marked and attached hereto as Annex “C”;
7. That as member of the Philippine Bar, his Roll Number is
53127 and his Integrated Bar of the Philippines Membership Number
is 718680. Marked and attached as Annexes “D” and “E”
respectively, are Certificate of Membership in the Philippine Bar
indicating his Roll of Attorneys Number and 2007 IBP Official Receipt
indicating his IBP membership number;
8. That he is of good moral character as certified to by at least
two (2) executive officers of the Integrated Bar of the Philippines,
Iloilo Chapter. Marked and attached as Annex “F” is said
certification to be taken as integral part hereof;
9. That he has paid the required fees for the filing of this
petition. Marked and attached as Annex “G” is the proof of

payment for the filing of the petition to be taken as integral part
hereof;
10. Three (3) passport-size color photographs with white
background taken within thirty (30) days of this application, signed
by him at the bottom part of the photographs are likewise attached
hereto as Annexes “H”, “H-1”, and “H-2”, and submitted as one of
the supporting documents hereof;
11. That as a practicing lawyer since April 2007 and also an
Associate of the Law Firm of Duremdes Nava & Duremdes, he now
maintains a regular office at 2 nd Floor, A & L Building, E. Lopez Street,
Jaro, Iloilo City with telephone numbers (033) 3203935 and (033)
3208519. Marked and attached as Annex “I” is the sketch location
of the office to be taken as integral part hereof;
12. That his specimen signatures are likewise hereto attached
in a separate paper as Annex “J”, “J-1”, and “J-2” to be taken as
integral part hereof;
PRAYER
WHEREFORE, premises considered, it is most respectfully
prayed of this Honorable Court to give due course to this present
petition and that petitioner be appointed as Notary Public in the City
and Province of Iloilo;

Other reliefs just and equitable under the premises are likewise
prayed for.
