Dear Nominee,

Congratulations! We are pleased to inform you of your election to membership in The International
Honor Society of Phi Kappa Phi, Chapter 045 (University of the Philippines) in recognition of your
excellent academic record.

Phi Kappa Phi has more than 1 million members worldwide and 300 chapters located in university
and college campuses throughout the United States, Puerto Rico and the Philippines, and is one of the
oldest and most respected academic honor societies. Those invited to membership include the upper
10% of seniors and 7.5% of juniors, along with outstanding graduate students, faculty, professional
staff and alumni.

Phi Kappa Phi is an honor society that recognizes superior scholarship in all academic disciplines. It
engages this community of scholars in service to others and upholds academic integrity. We hope
that you will accept our invitation to formally join the society. In order for you to become a member of the
society, payment of the induction fee and physical presence during the induction ceremony is required. If you
cannot accept our invitation now, you are welcome to join the society in the future.

The induction ceremony for new members will be held on Monday, 22 May 2017, 3:00-5:00 PM at the
UP Film Institute in U.P. Diliman, Quezon City. However, you must be at the UP Film Institute at
1:00 PM for refreshments for the inductees and attendance check which starts at 2:00 PM. Proceed
directly to your assigned marshal based on your last name. Only those who register from 2:00 PM –
3:00 PM will be called on stage to receive their certificates and pins. Processional starts exactly at 3:00
PM; those who fail to register on time will not be allowed onstage. Attire is strictly Filipiniana for the
inductee and formal for the parents/guests. You are entitled to two (2) guests only. In no case may
any of your guests be below 14 years old.

 INSTRUCTION SHEET

FOR ELECTED CANDIDATES FOR MEMBERSHIP IN THE

HONOR SOCIETY OF PHI KAPPA PHI
CHAPTER 045 (UNIVERSITY OF THE PHILIPPINES)

To qualify for full membership and be inducted into the society, please take note of the following procedures that
you must undertake:

1). Fill up the necessary and required information in the attached Permanent Record of Election (PRE) form.
Print two (2) copies of the form.

2). Pay the induction fee amounting to Php 4,500.00 to the Treasurer or Executive Assistant at the PKP Office
located in Room 329, National Engineering Center (Juinio Hall), Agoncillo Street, U.P. Diliman, Quezon City
during office hours, Monday – Friday, on or before May 12, 2017. This will cover the cost of the induction
ceremony, a Phi Kappa Phi emblem pin, a U.S.-prepared membership certificate, one year subscription to the Phi
Kappa Phi journal, one year international membership fee plus remittance costs, and one year local chapter
membership fee.

1. For Check Payment - payable to Phi Kappa Phi Honor Society Inc.

2. For Bank Deposit- Deposit your payment at any branch of Banco De Oro. Please note the following details.
Account name: Phi Kappa Phi Honor Society Inc. Peso Savings Account number: 3570131658. Scan the deposit
slip together with your PRE.
Please put your name and college in the deposit slip located at the Payor’s Name line item.
Example. Ma. Victoria Y. Bayno- College of Engineering, UP Diliman

Do not forget to bring the two (2) printed copies of the PRE with you when you pay to the Chapter Treasurer or
Executive Assistant. Have the Treasurer/Executive Assistant stamp “PAID” on the lower right hand portion of
the form and submit one (1) copy of the form. Do not forget to email electronic copy of PRE to
executiveassistant@phikappaphi.up.edu.ph. It will be sent to the PKP headquarters in the U.S. for you to be
included in their database so you can receive relevant membership materials. Retain one (1) copy for your file.
An Official Receipt will be issued to you.

3). You may also opt for Local Chapter Lifetime Membership by paying an additional fee of Php 3,000.00.
Payment of local lifetime membership fee is optional.

The annual membership renewal fee is $50.00 for international membership in the mother chapter and Php
1,000.00 for the local chapter and is required to be an active member and receive the full benefits of membership
for one year. Active-for-life membership to the mother chapter is $320.00.

4). After payment, fill-in the remaining details of your PRE form, namely, Official Receipt Number, Amount and
Payment Received By. Email back the e-copy of the PRE form to the Executive Assistant to
executiveassistant@phikappaphi.up.edu.ph. This will ensure that you will be included in the official list of
inductees for 2017. Otherwise, you will not be allowed to enter the induction hall. Non-submission of the e-
copy would mean no induction for you.

5). In case you are unable to join the society this year but are interested in joining the Honor Society in the
future, send the e-copy of your PRE form to the e-mail address executiveassistant@phikappaphi.up.edu.ph with
the subject title “unable to join now but interested to join in the future” in order for us to keep you on file and
be able to provide you with details of succeeding induction rites.

6). If you are unable to join because of lack of funds for payment of the induction fee and belong to Bracket E,
you may wish to email the Executive Assistant (executiveassistant@phikappaphi.up.edu.ph) to apply for a pay
forward scheme c/o the Phi Kappa Phi Foundation.

7). If necessary, make arrangement only with the official photographer, Mr. Solano of EXPO, just before the
ceremony. Their representatives will wear official ID’s.
