Background: Hypertension is classified as The Sillent Killer (silent killer). For


hypertension cases obtained from RSUD Ade Muhammad Djoen Sintang hypertension
incidence in 2016 as many as 114 cases and in 2017 experienced an increase of as many
as 141 cases, most cases occurred at age 45 years and above that is 118 cases.
Objectives: To know nursing care in hypertension clients include assessment, diagnosis,
intervention, implementation, and evaluation of nursing.
Methods: Interviews with clients and family, client record records
Outcome: After nursing care for 3 x 24 hours blood pressure decreased from 170/100 mm
Hg to 150/85 mm Hg, decreased intensity of headache, neck and shoulder tense, client
90% of activity can be done alone,
Conclusion: Cooperation between health teams, from 6 diagnoses, high risk of decreased
cardiac output, activity intolerance, less nutritional change and less problematic
knowledge partially resolved
