 Letter from parent /guardian to support the need for financial assistance addressed to:

 Photocopy of Parents latest Income Tax Return (ITR) or an affidavit of non-filing of ITR (stating the job and
estimated monthly income); for OFW – Photocopy of Contract of Employment
 Certificate of Indigency (Barangay)
 Photocopy of 4th year high school grades (for freshmen)
 Letter of recommendation for financial assistance from the high school principal or guidance counselor.
