Dear Mayor,

The undersigned employee of this municipality wishes to request for your

approval in connection with my desire to continue my studies at (school) taking up
Bachelor of Science in Computer Science starting this school year.

I am presently assigned at COMELEC Office as a Computer Encoder, given my

desire to serve for the betterment of LGU of this municipality, for this reason, I want to
pursue and finish my course hoping that someday I will have the chance to help and
impart my knowledge in this municipality. If condition warrants, I promise to comply
with the eight working hours a day.
