AFFIDAVIT OF CONSENT AND SUPPORT

2. That I am giving my consent for my child to travel to Japan and accompanied

3. That I, together with his above-mentioned uncle, am sponsoring his trip and
that I am financially capable to support his stay abroad;

4. That I guarantee that he will not violate and will faithfully abide to any
existing law, rules and regulations of Japan;

5. That I am executing this affidavit to attest to the truth of the foregoing facts
and in compliance to the requirements of my child’s application for travel
clearance with the Department of Social Welfare and Development (DSWD);
