Dear Sir/Ma’am,

This is to inform you that I want to close my account in your institution due to
some personal reason.

Hence, I request you to please close my said account and hand over the total
amount that I had been deposited since March, 2019 through salary deduction. I
enclosed some supporting documents to prove that I’d been deducted since I believed
that it will not automatically reflected in my account.
