Dear Sir,

GBC cooler#2 installation completed by MMD on 06/11/2018, PSM (B SHIFT) asked PM to permit to
open GBC#2 casing drains one of which was dummied condition and PM instructed PSM no need to
open the casing drains just purge through vents. After half an hour while checking the lineup PM came
again instructed PSM to open the casing drains. PSM opened the gas cooler drain (gas side) and
observed lot of chemical water and draining continued same thing was informed to MMD and PM. PM
instructed to continue purging and purging done throughout the day. Next day morning at 09:00 hrs. PM
instructed from the field no need purge further stop purging, PSM recommended to continue purging
and GBC discharge line inspection also. PM instructed PSM to charge the gas but PSM refuse to charge
and went to field observed that lot of water coming from GBC casing drains while purging same thing
has been recorded in video showed to PM and purging continued. Same day B shift MMD inspected GBC
discharge line and observed lot of water, dewatered through pump and purging done throughout the
day. Purging continued and again 10-liters of water removed from GBC discharge line on 9/11/2018 B
shift. Finally, after continuous purging on 11/11/2018 PM instructed PSM to charge the gas even though
0.5 to 1cm water is there in GBC suction line. After purging with natural gas GBC started at 14:23 hrs. on
11/11/2018 and GBC#2 tripped on stage 1 high vibration. GBC#2 started again asper PM instruction. If
we have started GBC on 6/11/2018 or 07/11/2018 on as per PM instructions, it would have been tripped
the GT and may be it would have been big outage also.
