Attached is the approved request of 8 pcs solid top table 8 feet long for library use the cost of
delivery charge is amounting to Nine Thousand Seven Hundred Pesos (P9,700.00).

In view of this may we request Mr. Norberto Cauzon and a service vehicle ISUZU NHR to go to
Manila to pick up the said table in the supplier warehouse on Saturday December 6, 2014.
