INSTRUCTIONS
(Read carefully before filling the form)

1. Examination Admission Form & Fee should read the University at the same time on or before the due
date of admission failing which fee due on the date the form is received will be charged.
2. Examination Admission Form and Fee for Part-I, Part-II & Part-III must be submitted separately. No
candidate will be allowed to appear in both parts on single Admission Form or single Roll Number.
3. University Registration number must be mentioned in the Admission Form otherwise Registration Fee
will be charged.
4. All candidates are required to attach three copies of recent white background photographs duly
attested on backside and one copy of National Identity Card/Domicile Certificate duly attested by the
Principal concerned.
5. Fee once deposited is neither refundable nor adjustable if the candidate is eligible.
6. Current DMC’s must be attached with the Admission Form.
7. Incomplete/Unsigned Form shall not be entertained.
8. Admission Fee by Money Order/Cheque shall not be accepted.
9. Every candidate must sign the Admission Form himself. In case of variation in signature from I.D. Card
the candidate will face Impersonation charges.
10. In Case of compartment the candidate having missed two consecutive chances (availed/un-availed) shall
have to appear afresh in the full examination.
11. Two different examinations are not allowed in one session.
12. Aggregate failed candidates can improve their aggregate not in more than three subjects.
13. A candidate having passed the lower level examination shall be required to appear in the higher level
examination within a period of three consecutive years, failing which the candidate will loose the right
of claim to the lower level result and shall appear afresh (in the lower examination only).
14. Possession of Mobile phone is not allowed in examination center. If any one found in possession, his/her
Examination will be cancelled, besides forfeiture of Mobile Set.
