________________________________ to participate in the Work Immersion Training Program that is a
requirement of Department of Education-REGINO MERCADO NIGHT HIGH SCHOOL.

We understand that the said skills training will cover 240 training hours, which will be conducted
in ____________________________, located at COLON STREET, CEBU CITY, FROM November 4, 2019 –
February 2020.

We are aware that the Work Immersion Teacher will conduct visitation and monitor the
performance of the trainee and that the immediate superior with the company will take all the necessary
precautions to keep the Trainee’s safe.

However, if despite all efforts taken, untoward incidents beyond their control occur, we will not
hold the company where our son/daughter took their Work Immersion liable
