Site Investigation Report (Fill Area)

ESB requested to carry out the SI work at borrow area of Countryview.

SIDSB will appoint 3rd party surveyor and back charge to contractor for as-built
drawing issuance.

Any transactions of scheduled controlled goods (Diesel/Petrol) by ESB required a

license issued by Ministry of Domestic Trade and Consumer Affairs (KPDNHEP) and
Bomba.

NISB required to prepare handing over process for settlement

gauge no 3 and 9 to ESB.

NISB required to continue the instrumentation monitoring 7 times

more according to total of 12 times instrumentation monitoring.

ICM requested by SIDSB to summarize Rod Settlement gauge

and settlement marker data/report no. 3 and 9 and handing
over to ESB

SIDSB informed ESB to maintain the incoming earth drain to silt

trap no 2 since the surface runoff will enter the existing earth
drain.

ESB required by SIDSB to maintain the silt trap no. 2 after handing
over

SIDSB advice ESB to make used of leaves as membrane filter.

ESB requested by SIDSB to omit instrumentation monitoring for

the inclinometer at P8.

ESB will maintain the existing earth drain since the water from silt

trap at L4 discharged to existing earth drain.

Construction gate will be located near to existing wash through

location.

ESB required to check back the survey work outside Plot L4 since
SIDSB intended to cut the green area besides L4 area.
