Dear Atty. Castro,

I’ve been an agent of ______________________ from _______________ to ____________________.

Last ______________, I have already tendered my resignation, soon after , I made a consistent follow up
on the status and even complied on my accountabilities. However, up to this date, my clearance and ELS
deactivation is not yet granted by my former insurance company ______________________________.

I felt that I need to invoke my right (s) to demand immediate action as being a life insurance agent is the
only source of livelihood to sustain the needs of my family.

Allow me to take this opportunity to share with you the series of events; communications and
confirmations that I cooperated for X months to ensure my clearance and ELS be out 30 days after my
resignation.

(State the chronological order of events, supported by any proof like emails text messages etc.)

Moreover, this is not compliant to Insurance Commission Circular Letter No. 2017-55: Guidelines in the
Submission of clearance as a Requirement in the Application for the Issuance of Agents License to Solicit
Business for another Insurance Company that highlights the following : Please see attached Circular as a
reference.

 If the request is through electronic mail, acknowledgment is within 24 hours from receipt of the
request and the reckoning of the 30-day period shall commence from the date the
acknowledgment electronic mail was received by requesting company
 Failure on the part of any insurance company to act on any request within the thirty (30) – day
period herein prescribed constitutes a justifiable reason for IC to act upon an application for
issuance of an agent’s license without the required written clearance.

Please give this matter your most preferential attention as you are only the governing body that can
protect the rights of agents like me.
