AFFIDAVIT OF WAIVER

WE, THE UNDERSIGNED, of legal ages, Filipino citizens and residents of Villa Concepcion,
Cauayan City, Isabela, after having been sworn to in accordance with law do hereby depose and
say:

1. That we are the surviving heirs of CHARITO V. SANTOS who died last January 21,
2019 at Cauayan Medical Specialist Hospital;

2. That CHARITO V. SANTOS is a grantee of Certificate of Amnesty by the National

Amnesty Commission issued by President Fidel V. Ramos;

3. That by virtue of said amnesty, benefits are given to his surviving heirs;

4. That we, JOJO R. SANTOS, JUDY R. SANTOS, JESON R. SANTOS, JEMALYN SANTOS
ROSARIO, JONALYN SANTOS TOLOGAN, JOYCE SANTOS JERUSALEM, and JENNIFER
R. SANTOS hereby waive our share in favor of JEFFREY R. SANTOS;

5. That in order to make legal the verbal relinquishment, we hereby execute this
affidavit allowing JEFFREY R. SANTOS to perfect his rights over the same;

6. That we have executed this affidavit with my voluntary act and deed without any
intent of speculation, that have read and understood the contents hereof are true
and correct to the best of our knowledge and belief;

ACCEPTANCE

7. That I, JEFFREY R. SANTOS, do hereby accept the said waiver.
