When our clinical days started in jinnah medicare school of nursing, I were assigned in
nephrology ward, when I went in ward, first I introduce myself with ward supervisor and
staff. I told them that I’m student of Post-RN B.ScN and I have assigned here for clinical,
today I will work here and find my objectives. In charge said to me that help to staff inflow
up. Staff said me that you check vital signs of male patients and I check female patients.
During follow up I observed that staff push BP cough on left hand where is atrioventricular
fistula. I object and informed her that we couldn’t check BP on fistula hand. She said me that
I am seiner and now you teach me. I said no, but it is not a correct technique that you check
BP on Fistula Site. I also informed to supervisor for that mistake. He agreed with me and said
Feelings:
At this time I look towards patient face, patient were feeling unhappy and also patient said
that due to disease I have already disturb and now you again hurt me and also you practice on
me.At the same time I feel that staff was also confused. And such mistakes are so difficult for
patients and really. I feel shame because such common mistakes will shows poor knowledge
Evaluation:
There were about more than 13 patients in the ward, it may be possible that mistake takes
place due to over workload, lack of knowledge or lack of attention/interest. But it was really a
big mistake. After this mistake other patients understands that we are new learners in this
field. And it’s so shameful for us. And also hurt patient, most of time nurses remain in touch

with patients so they have a lot of trust on nurses. Nurses should do interest and follow all
Action Plan:
After this incident I said to staff that please improve your knowledge and interest/attention
during your work. Because patient should be the first priority I thought that I will give
education to my juniors on it. And during my duty timing in case of need I will communicate
with my senior as such incident should not takes place. From this incident I learn many things
and I got experience for my whole life. After that incident I promise that i have to improve
my practical skills, knowledge and interest/attention, because mistakes are also the part of life
but we have to learn from them, and try to never repeat such a mistakes.
Conclusion:
It was really a big mistake. This incident gave us a lesson for making good clinical skills,
knowledge, interest and attention in our field. Because the nursing field has great importance
in the world we should care patients according to their need and try to build a good
relationship between patients and nurse. Such incidents are harmful for break relationship
