INCIDENT REPORTS

1. Who prepares the incident report?

In cases relating to students, the advisers prepare the incident reports.

In cases relating to teachers, school personnel and school properties, the principal
prepares the incident report.

2. What is the purpose of the incident report?

The purpose is to put into writing and keep a record of all incidents in the school involving
students, teachers, school personnel and school properties.

3. How is the incident report prepared?

A thorough investigation prior to the incident must be made prior to the preparation of
the report.

4. Who keeps the record of the incident reports?

The Record Officer has custody of all the incident reports.


HOME VISITATIONS

1. Who conducts home visitations?

Advisers and Subject Teachers conduct the home visits.

2. What is the purpose of home visitations?

The purpose is to:

a. inform the parents of the performance of their children.


b. encourage students to return to school.

3. How is home visitation conducted?

a. The adviser or subject teacher is given a home visitation slip signed by Mrs. Jenny J.
Bello as the Guidance Coordinator.

b. The adviser or subject teacher shall have the same signed by the parents.

c. The adviser or subject teacher shall take a photo as a mode of verification.

4. Who keeps the record of the home visitation slips?

The Guidance Coordinator has the custody of all the slips.
