Please submit your written explanation to the DISCIPLINARY MATTER/S being charged against you
described or narrated below within FIVE (5) DAYS from the date that this Notice was served to you.
DISCIPLINARY MATTER/S:
Failure on your part to give a written explanation and be present in any stage of the investigation shall be
considered a waiver of your right and management shall proceed in the bases of the facts gleaned from
the information, including documents and witnesses' testimonies, we have at hand. It will be to your best
interest then if you will fully cooperate with the investigation process. Otherwise, you cannot later on,
complain of lack of due process.
We also would like to remind you that in accordance with applicable law, Uber prohibits retaliation against
any employee who opposes unlawful harassment or discrimination. Any report of retaliation will be
thoroughly investigated. If a complaint of retaliation is substantiated, appropriate disciplinary action, up to,
and including summary dismissal, will be taken.
