Financial Consultants Pvt Ltd located at 201 202, Pride Icon Building,
Kharadi, Pune 411014.

I am appointed as the feild executive and working with Yes

Bank, While returning from customer feild Visit, Yes Bank Ltd Reciept
Book No R03331, containing Customer copy, Bank copy and Agency
copy Reciepts no R0333106 to R0333110. i.e. total 5 reciepts those are
unused were found missing on dated 18-06-2019. The Lost Report
bearing no 20935 at Pune City Police for said is already Reported.

This is to clarify that any of the above reciepts those are

Missing I deponent will be responsible for any miss use or wrongly filled
by any one. And will be ready to face any course of legal action that can
be taken against me.
