Dear NGCP:

You are welcome to assess and conduct evaluation on my contested lot within 20meters easement from
transmission line with buildup native and light material structure. Since the area is declared as
commercial space and titled as my personal property, I request that you accept the following waiver.

I, Manolito A. Gozalvo, hereby agree that:

National Grid Corporation of the Philippines are not responsible and not liable in any form for any
accidents that occur while I operate my business establishment as our source of income on my property
and as my primary investment. This includes accidents because of my negligence as well as from natural
disasters.

If any accident does occur, I cannot bring a lawsuit against National Grid Corporation of the Philippines,
nor I bring a lawsuit in the event of accident.
