RESOLUTION FOR THE FINAL ACCREDITATION OF THE DIVISION SCREENING AND

ACCREDITATION COMMITTEE (DSAC) FOR THE 2017 PALARONG PANLALAWIGAN

In the context of the final interview conducted from November 12, 2017 which was the last
part of the screening and accreditation process to accredit the official Athletic Delegation
(athletes, coaches, asst. coaches and chaperones) that can participate in the 2017 Palarong
Panlalawigan on November 20-25, 2017 in Masbate City, the DSAC deliberated on the findings
and rendered decisions thereof. It was unanimously voted and agreed upon by the majority of the
members of the disqualification of the following athletes, to wit:

1. JUAN H. DELA CRUZ, Wushu Boys, Cataingan – full eruption of all 3rd molars
indicating that he is more than the cut-off age of 18 years old. (Disqualified by the
Division Dentist).

This decision is final and executory.

We hereunto affix our signatures to the veracity and conformity to the above decision
without mental reservation that it was done with due process, in-depth deliberations and following
conscientiously and religiously what is embodied in the Revised Technical Guielines in the
Conduct of the 2017 Palarong Bicol which was based on DepEd Order No. 43, s. 2016 which is
the Implementing Rules and Regulations of Republic Act No. 10588 otherwise known as the
Palarong Pambansa Act of 2013.
