Dear Sir/Ma’am:
We, the researcher, from grade 12-Konrad Zusestudents’ of Agusan National High
School – Senior High School. Asking your help for our research paper as part of our
academic requirement in Practical Research 2- Qualitative- Quantitative Research
With this, we need your authorization/ consent for being one of our respondents in
conducting our research study. We will need 10 Grade 8 students of every
classroomwith the permission also from their advisers fromjunior high school and
whatever will be the result of the research will be kept confidential.
The research instrument that we will be using is a survey questionnaire that would
answer questions and/or related to our research topic entitled: Students Awareness on
Cyberbullying.
