mick is here with complaint of an ongoing sought throat for the last six-week although with what

he describe as a clorespeness incode in his voice, and tightness in his throat specialty with any
form of exertion.thesymptoms begin after he drinks coffee in the morning. although six weeks
ago he was exposed to an refrigerant for his working on his car although he was supposedly in
non-toxic. recently he also begins clearing his throat regularly. he did try thumb in past few
weeks but did not recall whater it improve his sympthom or not .his father was diagnosed with
_________later in life.he does continue to be physically active but it was really effective by it
shown in the breath feeling although he described higher up in the neck region. the symptom
did not seem to be worst when he lies down in the night.

function given order for CT soft tissue of the neck and chest. he would follow up in the next
three or four weeks.
