CONTRACT OF LEASE

KNOW ALL MEN BY THESE PRESENTS:

This contract is entered into and between:

JOSEPHINE C. SAUREZ, Filipino, of legal age, married and with postal address at 20 Mercury
Street, GSIS Heights Matina, Davao City, herein called the LESSOR;
and
LEO ARTEMIO A. PUERTOS, Filipino, of legal age, married and with postal address at
_______________________________, Davao City, herein called the LESSEE;

WITNESSETH
WHEREAS, the LESSOR is the owner of the residential house at Lot 3, Block 7 Phase 1, Lavista
Monte, Matina Pangi, Davao City;

WHEREAS, the LESSEE desires to lease and rent from the LESSOR the said house;

NOW, therefore, for and in consideration of the foregoing premises, the LESSOR hereby
leases to the LESSEE the semi-furnished house under the forgoing terms and conditions:

1. That this Contract of Lease shall be covered for the period of One (1) year from
March 1, 2019 to March 1, 2020, renewable at the option of the LESSOR;

2. That the LESSEE hereby agrees and obliges himself to pay without the, need of
demand to pay the monthly rental of EIGHT THOUSAND PESOS ONLY (P 8,000.00);

3. That upon the execution of this contract, the LESSEE shall pay to the LESSOR One (1)
month Advance of P 8,000.00 and One (1) month Deposit of P 8,000.00 total sum of
SIXTEEN THOUSAND PESOS ONLY (P 16,000.00);

4. That when the LESSEE failed to pay to the LESSOR for two (2) consecutive monthly
rentals, this contract shall be terminated without any further action on the part of
the LESSOR and that the LESSEE should voluntarily surrender the possession and
enjoy of the leased premises to the LESSOR.

5. That all the ordinary minor repairs within the premises that arise in the daily use of
the facilities, therein, shall be at the sole account and expense of the LESSEE,
without the right for reimbursement;

6. That the LESSEE shall not make any alteration or change something in the semi-
furnished rented house without the knowledge and consent of the LESSOR;
7. That the leased premises shall be used by the LESSEE exclusively for residential
purposes only ans prohibited to sub-leased the said premises;

8. That all charges of Water, Electricity bills and connection for Cable Subscription,
Telephone and Internet used within the premises shall be at the sole account of the
LESSEE;

9. That the LESSEE does hereby confess liability to LESSOR for any damage or
destruction to the contents listed on the schedule of furniture and appliances
attached hereto as Exhibit “A”, ordinary wear and tear excepted. The amount
of liability shall be the predetermined valuation of said furniture and
appliances as indicated on the attached Exhibit “A”, or in the amount
necessary to repair any partial damage to said contents. LESSEE shall also be
responsible and liable for repayment within ten (10) days to LESSOR, for
Lessor making repairs and/or replacements that may be required by injury or
damage to the leased premises or its contents, whether said damage was
caused by Lessee, Lessee’s family, or any other person or persons whom
Lessee invited or permits to be in or about the leased premises.

10. That the LESSEE shall be responsible for the observance of sanitary, home owner’s
association and electrical regulations required or imposed by the city government
authorities regarding the use and habitation of the leased premises;

11. That all account shall be fully paid by the LESSEE before he abandons the lease
premises;

12. That the LESSEE shal notify the LESSOR at least one (1) month in advance should the
LESSEE decide to abandon/vacate the leased premises;

13. That violation of any the above terms and conditions will produce ipso facto
(automatic) recessions/cancellation of the lease:

IN WITNESS WHEREOF, the parties have hereunto set their hands on this ___day of

ACKNOWLEDGEMENT
